Fatal Coronary Air Embolism during Positive Pressure Ventilation

We report the case of a 29 years old patient brought to emergency after road traffic accident. His blood pressure was 138/78 mmHg, pulse 119/min, respiratory rate at 24/min and Glasgow coma scale 6/15. The put on mechanical ventilation then transferred intensive care unit. A body CT scan performed which found multiple hemorraghic contusions subarachnoid hemorrhage in head. Lower right limb x-rays showed mid diaphyseal femur fracture surgically treated. 7 days surgery, suddenly developed hypotension, sinus tachycardia acute distress. What is most likely diagnosis this stage?. Considering condition, massive pulmonary embolism. Thorax immediately coronary air embolism circumflex artery as shown figure 1. high sensibility troponin level 8900 ng/l. Barotrauma has been estimated occur 0.5-38% critically ill patients [1]. Systemic eventually gas associated with positive considered rare, but catastrophic complication mortality rate.
